At The Facial Pain Center, we believe getting the care you need should be convenient and easy. Your first visit will begin with a consultation where we will listen to your concerns, understand your symptoms and provide a complete examination. We will determine a diagnosis, answer your questions, develop treatment options and provide actionable next steps.

Before your visit

Becoming a new patient is easy with our online registration system. After scheduling your appointment by phone, you will receive an email with instructions to register online prior to your visit. Completing the online registration will ensure a smooth registration process when you arrive so you can maximize your time with the doctor. However, if you prefer a paper form, you can download, print and complete our new patient registration packet by clicking the New Patient Registration Packet link below.

What to bring to your visit

To ensure you get the most effective and efficient care, please bring the following with you to your first appointment:

  • A list of all medications you are currently taking
  • Referral slip
  • Medical insurance card
  • Photo ID
  • Sleep study results (if applicable)
  • X-rays, MRI reports (if applicable)
  • New Patient Registration Packet  (only needed if you did not use our online registration system)

 

Important: A parent or guardian must accompany all patients under the age of 18 at the consultation visit.

Once you’ve registered as a patient, making future appointments is simple. As part of your first visit, we’ll discuss the right treatment schedule so you can plan ahead.
